Jaipur, 22 November 2022: Doctors at Fortis Jaipur successfully treated a 64-year-old female patient with an irreducible hernia and symptomatic gallbladder stone disease. Normally, in such conditions, two separate surgeries are required with the help of 7 ports (4 ports for cholecystectomy - a surgical procedure to remove your gallbladder and 3 for hernia repair), but in this case, the team of doctors led by Dr. Rajesh Sharma, Additional Director, Gastrointestinal, General, Bariatric and Minimal Access Surgery at Fortis Escorts Hospital conducted Concomitant Laparoscopic Hernia & cholecystectomy with 3 ports, all were placed on left side of the abdomen. Laparoscopic Cholecystectomy is a minimally invasive surgery to remove the gallbladder usually via 4 ports -2 in mid-line & another 2 on right side of abdomen. For Laparoscopic hernia repair, 3 ports are placed on the left side of the abdomen. This procedure has been conducted in this manner for the first time in Rajasthan as per the available medical literature /the best of the knowledge of the institute.

Upon admission at the hospital, it was found that a part of the patient’s intestine and liver ligament were stuck in the hernia. After a complete examination, the doctors decided to do both the surgeries simultaneously. Laparoscopic cholecystectomy has several benefits as compared to the traditional open surgery - Less pain, Lower risk of complications, Quicker recovery, Smaller wounds and scars.

Giving details of the case, Dr. Rajesh Sharma, Additional Director, Gastrointestinal, General, Bariatric and Minimal Access Surgery, Fortis Escorts Hospital Jaipur said, “We performed a diagnostic laparoscopy to operate on the hernia, followed by the laparoscopic surgery of the gall bladder. Both the hernia and gall bladder was operated at one go, without any infection. Had there been any spillage or infection, we would have to shift to an open surgery instead of doing it laparoscopically. During the process, the gall bladder was put inside the sterile bag and was taken out with the help of one of the three ports. After completing

the cholecystectomy, all surgical instruments were changed to prevent infection, and then a large size mesh was deployed to overcome the supraumbilical hernia.”

“Normally, in the laparoscopic cholecystectomy, 4 ports are created, two in the middle and two in the right side of the abdomen, and for the ventral hernia, three ports are created on the left side of the abdomen. Therefore, this patient would have required 7 ports ; keeping in mind the patient’s comorbidities, doing two subsequent surgical procedures would have been a huge risk thus we adopted a minimal invasive surgery.” Dr Sharma added.
