Heart Transplant Team Decision

This team, comprising experts from various departments, reviews the patient’s medical history, test results, and overall suitability for transplantation. Based on group consensus, the team may accept the patient for transplantation, provisionally accept the pending resolution of minor issues, defer the decision for future consideration, or categorically reject the patient.

Waiting for a Donor Heart

Upon evaluating and accepting patients for heart transplantation, organizations such as the National Organ and Tissue Transplant Organization (NOTTO) can enlist them on the national waiting list in India. This national networking organization in India aims to regulate organ donation and transplantation activities to oversee the National Human Organs and Tissues Removal and Storage Network, as mandated by Parliament. NOTTO strives to promote and facilitate organ transplantation nationwide through collaboration and data collection.

Medical Urgency and Waiting Time

Patients on the waiting list are prioritized based on medical urgency, with those facing life-threatening conditions receiving expedited consideration. However, the waiting time for a donor’s heart can vary significantly, ranging from weeks to several months or even years. During this waiting period, patients require ongoing medical care and support to manage their condition while maintaining readiness for transplantation.

Procedure

Heart transplant surgery, also known as orthotopic heart transplantation, involves replacing a diseased or failing heart with a healthy donor heart. The surgery typically lasts several hours and follows a well-defined sequence of steps:

  • Anesthesia and Incision: The patient is placed under general anesthesia to ensure their comfort and unconsciousness throughout the procedure. The surgical team then makes an incision in the chest to access the heart.
  • Removal of the Diseased Heart: The diseased heart is carefully excised from the patient’s chest, disconnecting it from the major blood vessels while preserving surrounding structures.
  • Donor Heart Implantation: The donor heart, procured from a deceased donor, is meticulously prepared and implanted into the recipient’s chest. The surgical team then connects the donor heart’s blood vessels to the recipient’s blood vessels, ensuring proper blood flow.
  • Cardiac Reperfusion: Once the donor’s heart is successfully implanted, the surgical team initiates cardiac reperfusion, allowing blood to flow into the new heart. This step is critical for restoring oxygenation and function of the transplanted organ.
  • Chest Closure: After confirming the proper functioning of the donor’s heart and ensuring hemostasis, the surgical team closes the chest incision using sutures or surgical staples.

Heart transplant surgery may also involve concurrent procedures, such as aortic valve replacement or heart valve replacement, to address additional cardiac issues and optimize overall heart function.

Postoperative Care

Following heart transplant surgery, patients require intensive postoperative care to facilitate recovery and minimize complications. This includes close monitoring in the cardiac intensive care unit (ICU), administration of immunosuppressive medications to prevent organ rejection, and rehabilitation to regain strength and mobility. Long-term follow-up care involves regular cardiac evaluations, medication management, and lifestyle modifications to promote heart health and ensure the longevity of the transplanted heart.
