Lung Transplant Considerations: What to Know Before Proceeding

Introduction 

Lung transplant refers to a surgery performed to take out a lung that is damaged or diseased and replace it with a healthy lung from a recently dead (deceased) donor—it is a life-sustaining treatment for individuals with late-stage lung disease. Studies show that lung transplantation is linked with a crucial improvement in quality of life. Read on to learn more about the eligibility criteria required for lung transplantation and the related evaluation process.

Who is Eligible for a Lung Transplant?

Patients are required to meet a wide range of criteria to be eligible for lung transplantation. The patient should:

  • Be physiologically 60 years or less for a double lung transplant and 65 years or less for a single lung transplant
  • Have a poor prognosis, with a life expectancy of 1.5–2 years
  • Not be suffering from any other life-threatening systemic disease
  • Have demonstrated absolute compliance with consuming medicines and following other medical advice and have superb rehabilitation potential
  • Exhibit stability of emotions and a practical understanding of the implications of organ transplantation
  • Have a nice social support system

Getting Ready for a Lung Transplant

The transplant evaluation process includes:

  • Psychological and Social Evaluation: This includes assessing stress, financial issues, and support from family and other loved ones.
  • Blood Tests: They are needed to help find a suitable donor match. It improves the possibility that the donor organ will not be rejected.
  • Diagnostic Tests for Lung Transplant Recipients:
    • Arterial Blood Gases (ABG)
      This test measures the amount of oxygen that blood can carry to the body tissues.
    • Pulmonary Function Tests (PFT)
      They measure lung volume and the airflow rate through the lungs.
    • Radiographic Studies (X-rays)
      A radiographic study needs the usage of X-rays. The most predominant is the chest X-ray. A chest X-ray (CXR) is a pain-free, three-minute process that takes an internal picture of the chest, including the lungs, ribs, heart, and the contours of the great vessels of the chest.
    • Computerized Tomography (CT) Scan
      Pictures of the horizontal slices of the chest are obtained in a CT scan, and a computer projects these pictures.
    • Ventilation Perfusion Scan (V/Q Scan)
      This test compares right and left lung functions. 
    • Electrocardiogram (EKG)
      Electrocardiogram is a ten-minute procedure conducted by placing six electrodes on the chest and one electrode on each of the four limbs. A recording of the electrical activity of the heart is achieved, providing information about the rate and rhythm of the heartbeat and how the heart is located in the chest, which helps assess any heart/cardiac damage.
    • Echocardiogram (ECHO)
      Echocardiogram is an ultrasound of the heart. It is performed to evaluate the impact of lung disease on the heart’s mechanics.
    • Radionuclide Ventriculography (RVG)
      This test evaluates the performance of both the right and left ventricles, the two main pumping chambers of the heart.
    • Barium Swallow
      This test involves taking X-ray pictures of the esophagus and stomach after swallowing a minute quantity of the contrast material.
    • Blood Specimens
      Blood samples are needed for both routine and specialized testing. Specimens are sent for liver function tests and evaluating blood chemistries, including potassium, sodium, cholesterol, triglycerides, and other electrolytes.
    • Other Preparations 

Several immunizations will be administered. These are to lessen the chance of infections impacting the transplanted lung.

Before a Lung Transplant

After the lungs of a compatible donor become available, the transplant candidate is asked to turn up to the transplant center to prepare for the surgery. At the same time, the surgical team examines the deceased donor’s lungs to make sure they are fit for transplant. If they are, the surgery commences immediately.

The procedures involve either a single or a double lung transplant. Each option has benefits and pitfalls, and the choice differs with the recipient’s lung disease and other factors. The cost of a lung transplant also depends on the procedure chosen.

To decrease the chances of rejection, the candidate or patient should be well-versed in the procedure, meet the eligibility criteria, and undergo a prior assessment.
