Understanding ACL Repair: Insights from Dr. Vinay Kumaraswamy

 What is the ACL?

The ACL is a crucial ligament in the knee that stabilizes the joint during activities like running, jumping, and changing direction. Injuries to the ACL are common in sports involving sudden stops, pivots, or impacts, such as football, basketball, and soccer.

 Symptoms of an ACL Injury

Common symptoms include:

  1. A popping sensation at the time of injury
  2. Severe pain and inability to continue activity
  3. Rapid swelling
  4. Loss of range of motion
  5. Instability or the feeling of the knee "giving way"

 

 Diagnosis:

Diagnosis typically involves a combination of physical examination and imaging studies:

  1. Physical Examination: Tests like the Lachman test and pivot shift test help assess the integrity of the ACL.
  2. Imaging: MRI scans are used to confirm the diagnosis and evaluate the extent of the injury.

 

 Treatment Options

Treatment depends on the severity of the injury and the patient's activity level:

Non-Surgical:  Rest, ice, compression, and elevation (RICE), along with physical therapy, may be sufficient for partial tears or less active individuals.

Surgical: ACL reconstruction is recommended for complete tears, especially in active individuals. The procedure involves replacing the torn ligament with a graft, typically taken from the patient’s own hamstring or patellar tendon.

 

 The Surgical Procedure:

1. Preparation: The patient is positioned, and the surgical area is sterilized.

2. Incision: Small incisions are made to insert an arthroscope and surgical instruments.

3. Graft Harvesting: A graft is taken from the patient’s hamstring or patellar tendon.

4. Torn ACL Removal: The remnants of the torn ACL are removed.

5. Graft Placement: The graft is positioned and secured in the knee using screws or other fixation devices.

6. Closure: The incisions are closed with sutures, and the knee is bandaged.

 

 Post-Operative Care

Recovery involves a structured rehabilitation program:

Phase 1 (0-2 weeks): Focus on reducing swelling, regaining range of motion, and gentle strengthening exercises.

Phase 2 (2-6 weeks): Increase strength and flexibility with more intensive exercises.

Phase 3 (6 weeks-6 months): Progressive return to activity, emphasizing balance, agility, and sports-specific drills.

 Return to Sports:

Most patients can return to their sports activities within 6 to 12 months post-surgery, depending on their progress in rehabilitation and the specific demands of their sport. It is crucial to follow the rehabilitation program closely and avoid rushing back to activities to prevent re-injury.
