A 33-year-old lady with high BMI and multiple comorbidities successfully underwent a life-changing surgery at Fortis Hospital, Nagarbhavi

Bengaluru, June 14, 2022: A 33-year female suffering from multiple comorbidities along with high BMI rate successfully underwent a Metabolic surgery at Fortis Hospital, Nagarbhavi, Bangalore. The lady is a case of morbid obesity with a weight of 125kg and a BMI of 52 (super obese). She was suffering from Metabolic syndrome which is a cluster of conditions that occur together and increase the risk of heart diseases, stroke, kidney disorders, etc. The components of this syndrome include obesity, increased blood pressure, high blood sugars, excess body fat around the waist, and abnormal cholesterol or triglyceride levels.

The lady had visited the hospital with complaints of progressive weight gain for 3-4 years along with pain in lower back, left sided knee pain, difficulty in walking long distances, bilateral varicose veins and PCOD. She had tried all possible ways to control and reduce her weight, but was not successful. She also had Type II Diabetes, Hypertension (high blood pressure), dyslipidemia (high cholesterol), Hypothyroidism, Obstructive sleep apnoea (OSA), Fatty liver, abdominal hernia and was on treatment with multiple medications for the same.

The team of doctors was led by Dr Ramraj V N- Consultant Surgical Gastroenterologist, Advanced Laparoscopy, Bariatric and Metabolic Surgeon at Fortis Hospital, Nagarbhavi.

Dr Ramraj V N explains, “This lady was a high risk patient considering all her multiple comorbidities. As she was suffering from Metabolic syndrome, we had to look at reducing her weight and at the same time controlling her diabetes as these were becoming the root cause for most of her other conditions. Therefore, we decided to consider Bariatric & Metabolic surgery for her. Metabolic surgery not only reduces weight and maintains this lost weight, it gives excellent long term control of Type II Diabetes Mellitus and also keeps all other components of Metabolic syndrome under very good check.

After thorough investigations and fitness approval from Dr Ramesh D K- Consultant-Internal Medicine, Dr Majeed Pasha- Pulmonologist and Dr Prabhakar C Koregol- Cardiologist, we decided to go ahead with this surgery. As a pre surgery preparation, she was put on a very low calorie diet (VLCD) for one week, she then underwent Laparoscopic Roux-en-y gastric bypass. She was started on oral clear liquids from first day after surgery and discharged after 3 days on a high protein liquid diet with simple pain killers. All her diabetic medications were stopped at the time of discharge and she was back to work after 1 week. One month after surgery, she is now on low calorie, high protein soft diet, has lost 10 kg, her sugars are under very good control despite stopping all her diabetic medications immediately post surgery, and has also reduced her anti-hypertensive medications.”

The prevalence of metabolic syndrome is high among urban young adults in India, and it increases with increase in body mass index (BMI). Increased BMI, smoking, and history of hypertension are an associated risk factor for metabolic syndrome. Healthy lifestyles should be encouraged, and young adults should be considered as an important group for cardiovascular risk reduction programs.
