Phases of Cardiac Rehab


Cardiac rehab consists of three phases, each designed to support patient recovery and long-term heart health:

  • Phase I: Clinical Stage or Inpatient Rehab


This phase begins while the patient is in the hospital following a cardiovascular event or intervention. The primary purpose is to assess the patient’s physical capabilities and readiness for rehabilitation. Therapists and nurses provide education through light exercises, such as range-of-motion activities at the bedside, to prevent deconditioning. The rehabilitation team also evaluates the need for assistive devices, educates the patient and family members, and aids in discharge planning. Patients are advised to rest until other medical conditions or complications have been stabilized.

 

  • Phase II: Outpatient Cardiac Rehab


Once the patient has been stabilized and medically released by their cardiologist, outpatient cardiac rehabilitation begins. This phase generally lasts 3 to 6 weeks but can extend up to 12 weeks. The patient undergoes a detailed examination to identify physical limitations, comorbidities, and activity restrictions. Individualized therapy includes education, counselling, a customized exercise program, and relaxation techniques. The goal of this phase is to help the patient become independent, make lifestyle changes, and prepare to return to daily life at home.

 

  • Phase III: Post-Cardiac Rehab


Phase III focuses on the patient’s independence and self-monitoring. Emphasis is placed on flexibility, strength, aerobic conditioning, and the evaluation of the patient’s return to an active life. Follow-up visits with physicians are recommended to monitor cardiovascular health, manage medications, and provide guidance on healthy lifestyle changes to prevent recurrence and achieve long-term heart health.
