Overview

A penis pump is a device that is used by men with erectile dysfunction (ED). Erectile dysfunction is a common and multifactorial condition that affects males who are more than 40 years of age. It is the recurrent and consistent inability to maintain the penis erection during sexual intercourse. It causes immense psychological effects on both the affected individuals and their partners. 

Oral medications, medications at the penile tips, penile injections, penile implants, and penile pumps are some of the treatment options for ED. Penis pumps are amazing nonsurgical choices for ED which can bring artificial erection to the penis. They are also called vacuum pumps. 

About Vacuum pumps

Vacuum pumps use negative pressure to increase blood flow to the penis. These are of two types. They are Vacuum constriction devices (VCD) and vacuum erectile devices (VED). VCDs have external constriction rings to prevent blood outflow and maintain erection for a long time. VEDs do not have the constriction rings and are meant to increase the oxygen supply to the penis.   

About Pump

Penis pumps are external devices which can be used without any surgery for ED. It has an outer cylinder or a plastic tube, a hand or battery-operated vacuum pump, and an elastic band or tension ring. 

The outer cylinder is placed over the penis by closely approximating it to the body to create an airtight seal. A small hand-operated battery is used to create a negative pressure around the penis by engorging it with blood supply. An elastic band is kept around the penis to maintain the created erection.

 Releasing the vacuum maintains the erection for up to 30 minutes. It makes the penis hard and keeps it hard for penetrative sex in ED. It can also be used for pleasure. 

Indications

Penile pumps are indicated for almost all causes of ED. They help to create an artificial erection that helps in sexual intercourse. It is used as a second-line therapy among various treatments for ED. It is also indicated in the elderly who participate in occasional sexual intercourse. It is also indicated as an add-on therapy along with other treatment strategies. It is also indicated in individuals where natural potency must be preserved. 

Contraindications

Penile pumps are not indicated in men with spontaneous, intermittent, and prolonged penile erection. It is also not indicated in those with severe penile anomalies by birth or those acquired later in life. It is not advisable in individuals with high spinal cord injuries, neurological diseases, or degenerative joint diseases with poor manual handling. It is also not advisable for those on anticoagulants as there is a risk of developing blood clots. 

Advantages

Penile pumps have many advantages: 

• When used properly, they help men to get enough erection for sex. 
• They are safe and have relatively fewer complications than other modes of treatment. 
• They are affordable.
• They are nonsurgical options of treatment.
• They are present on the body surface but not inside the body. 
• They can be used along with other treatments used for ED.
• They can help with natural erection after prostate cancer surgery or radiotherapy.

Before using Penile pumps: 

Before using penile pumps, it is always advisable to meet a healthcare provider (HCP) to confirm ED. HCP may provide other treatment options if the ED is due to any other causes. The choice of prescribing the penis pumps depends on medical history like previous illnesses, and previous surgeries especially involving the penis, testes, or other male reproductive organs. One should inform the HCP of the various medications that are taken like blood thinners or any supplements. History regarding previous treatments for ED will also be enquired and a physical examination will be done to confirm the ED. A rectal examination will be done to check the prostate gland. 

Selecting a Penis Pump:

Select a penis pump as prescribed by an HCP based on the model suggested. Confirm the presence of a vacuum limiter by buying one. This prevents excessive pressure from damaging the penis. 

Using a penis pump

Before using a penis pump, make sure the pubic hair is removed so that the hair will not get caught in the pump. Follow the instructions given by the manufacturer for disposing of the pump. While using the pump, wash the hands and place the plastic tube over the penis. 

Using a hand or battery pump, draw air out of the tube to create a vacuum. This vacuum draws blood into the penis, thus creating an erection.  Once there is an erection, place the tension ring at the penis's base to maintain the erection for a long time. After getting the erection, remove the vacuum device. The erection lasts long enough to finish the sexual intercourse. The tension ring should not be in place for more than half an hour as a longer duration of the ring cuts off the blood supply causing damage to the penis. 

Effectiveness of Penis pumps

Penis pumps are effective, and over 90% of men have experienced functional erection with them. If used correctly, they can create a good erection. 

Side effects and drawbacks

One of the side effects of penile pumps is petechiae which are pinpoint red dots due to bleeding under the skin. They also cause bluish discoloration and paleness of the penis when the tension ring is applied too tight. They can cause painful semen ejaculation. 

The other drawbacks include unnatural erection of the penis, which may not be comfortable to some. It can be challenging to use until practiced properly. Penis may feel numb and cold for some individuals. 

Some complications like urethral bleeding, ulcerations of the skin, hematomas, and gangrenes can develop occasionally. 

Conclusion: 

Penis pumps are established second-line treatment options for men with erectile dysfunction. They are the nonsurgical vacuum devices placed over the skin for erection. They are relatively safe, affordable devices that can create an erection sufficient for sexual intercourse.
