Vasectomy reversal

Vasectomy reversal (VR) gives men a chance to fatherhood after previously opting for a vasectomy. While vasectomy is intended to be a permanent contraceptive method, it's common for men to reconsider and want children later in life. About 20% of men feel this way, and in the United States, around 2–6% of those who have had a vasectomy eventually pursue a reversal.

Vasectomy reversal is a specialized micro surgical procedure that restores fertility by reconnecting the vas deferens, the tube that transports from the testicles. The procedure success depends on various factors such as the time since the vasectomy and health of both partners. For those considering VR, understanding the entire process is essential. This includes preoperative evaluation and counselling, the surgical techniques involved, and the potential risks and benefits.

Rationale for Vasectomy Reversal

The procedure of Vasectomy reversal (VR) is performed in men who wants to restore their fertility which previously had vasectomy but later decide they want to have children. Although vasectomies are intended to be permanent, about 20% of men change their minds and desire to father children.

Procedure Details

VR is performed using advanced microsurgical techniques and involves two main procedures: vasovasostomy (VV) and epididymovasostomy (EV).

Vasovasostomy: The doctor stitches the separated part of vas deferens back together in this treatment. This method is chosen when the ends of the vas deferens can be easily rejoined.

Vasoepididymostomy: This technique involves attaching the vas deferens directly to the epididymis, the small organ at the back of the testicle where sperm is stored. Vasoepididymostomy is more complex than vasovasostomy and is typically selected when a vasovasostomy is not feasible or unlikely to be successful.

Preoperative Evaluation and Counselling:

Female Evaluation: It’s important to assess the fertility potential of the female partner. Women over 35 should have a fertility evaluation because pregnancy rates decline with age. Women over 40 have even lower chances of getting pregnant after VR.

Male Evaluation: The man’s fertility history, previous pelvic surgeries, and use of anabolic steroids or testosterone supplements are reviewed. A physical examination checks the vasectomy site, the condition of the vas deferens, and the health of the testicles.

Preoperative Counseling: Counseling involves discussing the chances of success based on various factors like how long it’s been since the vasectomy. A predictive tool (nomogram) helps set realistic expectations by considering patient age, time since vasectomy, and other factors.

Surgical Procedure:

Anesthesia and Positioning: General anesthesia is preferred for its comfort and because it helps with the precise nature of the surgery. Alternatively, neuraxial anesthesia with sedation can be used. The patient lies on their back, and the surgical area is prepared.

Operative Technique: If Vasovasostomy (VV) is preferred small incisions are made to access the vas deferens. The blocked section is removed, and the fluid from the testicular end is examined. If VV is appropriate, the ends of the vas deferens are carefully stitched back together using a two-layer technique.

If VV is not suitable, Epididymovasostomy is performed, connecting the vas deferens directly to the epididymis.

Intraoperative Sperm Extraction: Sperm extraction and freezing might be provided during VR to avoid future sperm retrieval if the reversal isn’t successful. This is especially useful for couples with lower fertility potential or if a bilateral EV is needed.

Benefits of Vasectomy Reversal

The important benefit of VR is the potential to restore fertility and achieve natural conception. Other benefits include:

  • Successful VR eliminates the need for expensive and invasive procedures like in vitro fertilization (IVF) or assisted reproductive techniques (ART).
  • Unlike ART, which often requires multiple attempts, a successful reversal can provide ongoing fertility with permanent solution.
  • Many men and couples find emotional satisfaction in being able to conceive naturally.

Risks of Vasectomy Reversal

Like any surgery, vasectomy reversal comes with risks, including:

  • The surgical site might get infected, requiring antibiotics or further treatment.
  • There’s a risk of bleeding and blood pooling at the surgical site.
  • Scar tissue might form and block the vas deferens again.
  • Rarely, the blood supply might be damaged, causing the testicles to shrink and cause testicular atrophy

Procedure Duration

Vasectomy reversal is a more complex and technically demanding procedure than the original vasectomy, which typically takes about 15 minutes. The reversal surgery, which requires the expertise of a skilled microsurgeon, can take up to three or four hours. This procedure is often done on an outpatient basis under general anesthesia. In some cases, insurance may cover the cost of a vasectomy reversal.

Time of Recovery from a Vasectomy Reversal

Recovery time after a vasectomy reversal can vary depending on the individual, but generally, patients are able to move around shortly after surgery. Most men can resume non-strenuous activities within 24 to 48 hours. Given the complexity of the procedure, it is usually recommended that men take about a week off from work. Sexual activity should be avoided for two to four weeks following surgery to enhance proper healing.

Postoperative discomfort is expected but can typically manage with over-the-counter pain killers or prescribed pain medication if necessary. Applying an ice pack to the testicles can also help alleviate discomfort. Most patients find that significant pain subsides after the first 48 hours.

Efficacy of Vasectomy Reversal

The success rates for vasectomy reversal are quite high when performed by an experienced microsurgeon. According to Dr. Honig, the success rate is between 80 to 90 percent for the return of sperm in the ejaculate after the procedure. While this high success rate does not guarantee pregnancy, many couples are able to conceive naturally following a vasectomy reversal.

Conclusion

Vasectomy reversal is a practical option for men who wish to regain fertility after a vasectomy. It involves detailed microsurgical techniques and thorough preoperative evaluation and counseling to set realistic expectations. With proper surgical expertise and postoperative care, many men can successfully produce sperm and have the potential to achieve natural conception Despite the risks, the potential to conceive naturally and avoid ART makes VR an appealing choice for many couples.
